The Acute Support Social Work Department - Finalist for the 2019 Chief Minister’s Inclusion Awards
The Acute Support Social Work Department at The Canberra Hospital is a finalist for Excellence in Inclusive Services and Disability Support at the 2019 ACT Chief Minister’s Inclusion Awards!
With the transition to the National Disability Insurance Scheme (NDIS), the Acute Support Social Work Department saw the need for a specific social worker to help support consumers with complex disabilities navigate both the health system and the National Disability Insurance Agency (NDIA). An NDIS Social Work Lead role was established, leading to the development of pathways with the NDIA and other internal and external services were established to assist consumers with building their support plans, and shorten time spent in a hospital. People were able to leave the hospital for a home of their choosing with the supports they decided upon.
